Required Permission Forms

  • For participants under 18, a parent or guardian will need to fill out the Youth Participant Drop-off and Pick-up Form (Google Form) before the first rehearsal.
  • All Participants and Parents/Guardians: Please come a few minutes early to the first class to fill out the Paper Packet. We will provide blank copies.

College Students

College students are welcome to join Community Music School programs that are open to their age range. CMS lessons are a great opportunity for adult students who are not music majors or minors to pursue their musical interests.
